- Canadian birth certificates (all provinces and territories)
- Verification of status or record of landing (for clients born outside of Canada)
- Agency assistance forms for OHIP
The ID Team doesn’t assist with:
- Passport
- Driver’s License
- Permanent residence card
- Citizenship Card
Clients who lack secure housing or rely on the emergency shelter system often resort to sleeping on the streets. It is extremely difficult to find a secure place to store identification… Street Health’s ID Safe provides secure storage for ID documents as well as an address for mail delivery.

Harm Reduction Approach:
Street Health’s harm reduction focus ensures that people who use substances are treated with respect and without stigma and that substance related problems and issues are viewed systemically. Street Health provides services in a non-judgmental way regardless of substance use.
Trinity Community Hub
Trinity CommUNITY Hub is the latest iteration of the Church of the Holy Trinity’s social justice endeavours.
The core of the program is weekly meals, a clothing bank and access to harm reduction supplies on Tuesdays and Wednesdays from 12 to 1:30 PM. Building community and centering a diversity of marginalized and stigmatized people.
The Hub also collaborates with a variety of other programs to provide wrap around supports to our community. Including mobile health care, art therapy, chess club, the homeless memorial, and more!
